The rural areas have been pretty lucky so far, but it looks like places where there are clusters of densely packed people working or living together are at risk.

Hong Kong was doing well but more recently had an explosion of cases among foreign workers living in dormitory like conditions. I'll try to get the links to these.
A group of nuns also suffered an outbreak, and of course the retirement homes.
In Michigan our hotspot started in Detroit, where the airport, at least for a while, was the only one allowing flights in from China, (forced to allow?). People up north in the UP were begging folks wanting to come to their summer cottages to bring their own groceries and stay away from the year round residents. The good thing about the stay home orders is those folks were protected for a while. The tiny rural hospitals would not be able handle a bunch of sick folks. However there are several large prisons up there too, which could seed an outbreak. Our fruit farmers in northern lower Michigan rely heavily on seasonal workers that live in "camps". With borders closed, the workers may not be available, but even if there are any, the camps with less than stellar hygiene would allow the virus to spread. This is a problem that a general solution would be good. Maybe some kind of public works projects would provide workers. Then there are the logistics issues for keeping them safe... not an easy problem to solve
