It might be an impossible fitting! Stand corrected a few post down. OD of 3/8" pipe is 0.675" 16mm = 0.630" Might be best to go the electrical gauge route. 16mm by 1/4" NPT looks to be as big as you can get. Example: 16mm - 3/8 NPT Reducer Bushing

Three o-rings are used to seal the metal plate to the manifold. If the bolts are already snug, tightening further is unlikely to help. Perhaps one of the o-rings got pinched during the assembly process.
