Are pumps simply plug and play, in that if the shims are kept the same pumps can be swapped out without any other adjustments being necessary?
Yes the shims are set to the block and not the the pumps, so as long as you use the same shims the timing will not be affected.

Would valve lash have anything to do with it?
Yes it could, but safe yourself some abuse and wasted time and do a compression test.
Low compression will make them act exactly how your are describing (on and off starting).
Colder the temps the harder they are to actually fire even with good glow plugs.

If the pressures are good they we can narrow it down to bad injectors.
