How does this cylinder piston damage happen?

So how on Earth can this backhoe swing cylinder get damaged in this way?
I tore it down to reseal for a leak and well more $$$ needed. Lol
The parts manual shows a bushing in the threaded end piece (gland?), does anyone know if I can replace that or does it get reamed to final dimensions after install or such?

It would really help if you told us what this cylinder fit.
It looks like the end of the piston broke and blew out but there may be more involved.

That's a very Unique cylinder!

It looks like the control end is broken off.
I do believe the control end keeps the piston from slamming into the bottom of the cylinder.
And that part probably caused the break in the piston.

Did it blow a hydraulic line under use?

It looks like you will have to replace the entire cylinder, because I'll bet there is damage inside the piston bore.
