BX25D wheel fell off

This winter I purchased a set of R4 (agi) tires. Took turf tires off and replaced them with R4. on the left front wheel we noticed we had to tighten the lug bolts continuously. The lug holes on rims were getting bigger. We lost 3 lug bolts. Finally the wheel fell off and the 2 bolts we had were on the flange and tight. The rim holes were just to big for the bolt. Replace with turf tire. Only 2 holes on wheel flange were good to screw in bolt other 3 holes on flange (wheel hub) wouldn't take the thread on the bolt. Anyone think they may know what's going on? What would do this?

The holes were wallowed out from running loose. Seen it many times.

Running it with one lug nut missing let alone 3 is really setting yourself up for issues.
You need to replace the lug bolts before you do more damage!
