L3000DT clutch....trans....issues

So, this is my first post....and I have a feeling I'm going to be asking a lot of questions and I'm hoping someone will shine some light to some of my problems....

First I had the clutch go out at 635 hours then the high/low lever broke 5 hours after the clutch was repaired.
Fixed the high/low lever then the pto clutch would not disengage...I tried to adjust the clutch with no luck...
I had to split the tractor again and found two of the three bolts that ride on the throw out bearing broken...

Now my questions are.... I have done everything to factory settings, from setting the inner clutch the 3/4 turns to the three bolts on the pressure plate set at .495 and the clutch peddle set at at 1 1/8 (I know it supposed to be 20-30mm)
Now every once in a while the clutch will not disengage and I have to shut the tractor off and pull it out of gear, then take it out of high or low.
Also I forgot to add that I added two quarts to the 7 gals of trans fluid...

Brand new pressure plates and clutchs from kubota.
After two of the three bolts broke I reused the three old bolts from the old pressure plate. They did have a little wear to them, so I rounded them back out on the bench grinder then repolished them so they had a smooth finish.

Did you replace the throw out bearing?
Did you surface the flywheel?

Yes, and I replaced throw out bearing, bearing hub and when I split it the second time I even replaced throw out bearing again and fork and wrist pins.

Now one thing I forgot to share was that the last time I split the tractor I reused the same trans fluid but it only had maybe 6 hours and looked clean.
I also used a magnet and had the rear wheels off the ground spun the trans and fished out the broken high/low arm.
