Does anyone know if I can add or convert an L245DT to have a creeper gear?

You could if you could find the parts, but the chance of that is about the same as you winning the big lottery!
That was a very rare option on that tractor, or any US model tractors, it was only a real option for trencher attachments and rice field rototillers.

Some older International tractors had creapers but you have to be careful because some of them also had reverse ptos. I'm sure other tractors did as well I'm just not familiar with any
