It's easy to tell- there aren't that many holes in the axle for adjustment- if the tire looks fairly centered with the fender, then you should have one more step to the outside.

Tight. You need to torque them- sorry that I can't remember the amount, but it is up there. They need to be tight, so that the hub doesn't wiggle on the axle.
